The medical records office at Carson Valley Health is located on the third floor of our CVH at Ironwood building, 897 Ironwood Drive in Minden. Office Hours are:
Monday - Thursday 7:30 am to 5:00 pm; Friday 7:30 am to 4:00 pm
All requests for medical records require a medical release authorization form that can be obtained from the medical records office during business hours or can be downloaded from our website.

All requests for patient health information must be authorized by patient's signature or patient's representative. A copy of your drivers license or another valid form of identification is required.
